Physiotherapy Following Emergency Abdominal Surgery

چکیده
Physiotherapy following elective abdominal surgery has been well documented, but following emergency abdominal surgery, despite poorer outcomes and increased complication rates, physiotherapy interventions for this patient group remain largely uninvestigated. The most common complication following upper abdominal surgery is the development of a post-operative pulmonary complication (PPC). Risk factors for the development of PPCs include duration of anaesthesia, emergency upper abdominal surgery, current smoker status, respiratory comorbidities, obesity, increased age and multiple surgeries. Physiotherapy interventions aim to prevent or remediate PPCs and post-operative complications associated with the sequelae of immobility such as venothrombotic events and to facilitate recovery from surgery and a return to normal activities of daily living and function. Physiotherapy interven‐ tions after major surgery include early mobilisation and respiratory physiotherapy techniques. Respiratory therapies include deep breathing and coughing exercises, positive expiratory pressure devices, incentive spirometry and non-invasive ventila‐ tion. Early mobilisation has been demonstrated to be safe and efficacious following elective abdominal surgery and for patients who are critically ill. This chapter reviews the evidence in these populations and propose that, until further studies are available to direct care, this evidence is extrapolated to patients following emergency abdomi‐ nal surgery. As abdominal surgery impacts on physical recovery and health-related quality of life, post-discharge rehabilitation programmes may improve long-term outcomes; however, rehabilitation following major cavity surgery is in its infancy. This chapter investigates post-operative rehabilitation research to date in this population in an attempt to determine the effectiveness of such programmes and make recom‐ mendations for future practice. © 2016 The Author(s). CONTEXT AND OBJECTIVE Abdominal surgical procedures increase pulmonary complication risks. The aim of this study was to evaluate the effectiveness of chest physiotherapy during the immediate postoperative period among patients undergoing elective upper abdominal surgery. متن کاملContinuous positive airway pressure reduces respiratory complications following abdominal surgery.
Objective: To review the evidence as to whether continuous positive airway pressure (CPAP) reduces postoperative pulmonary complications compared with standard care in patients undergoing major abdominal surgery. Data sources: OVID version of MEDLINE, EMBASE, CINAHL, and Cochrane Central Register of Controlled Trials, searched to November 2005. This search was supplemented by mail and phone fol...
متن کاملThe quantity of early upright mobilisation performed following upper abdominal surgery is low: an observational study.
QUESTIONS How much upright mobilisation, particularly uptime, is performed in the first four days following upper abdominal surgery? In what part of the day is the greatest uptime achieved? Is length of stay related to uptime? Is there any difference in uptime in terms of postoperative factors? DESIGN Prospective observational study. PARTICIPANTS Fifty patients who had undergone upper abdom...
متن کاملPhysiotherapy interventions to prevent postoperative pulmonary complications following lung resection. What is the evidence? What is the practice?
ABSTRACT Following major surgery pulmonary complications are an important cause of postoperative morbidity, contributing to significant increases in length of hospital stay, overall hospital costs and patient discomfort. Physiotherapy aims to prevent and treat pulmonary complications and a number of high quality studies have investigated the efficacy of various physiotherapy interventions in ma...
متن کاملChest physiotherapy with positive expiratory pressure breathing after abdominal and thoracic surgery: a systematic review.
A variety of chest physiotherapy techniques are used following abdominal and thoracic surgery to prevent or reduce post-operative complications. Breathing techniques with a positive expiratory pressure (PEP) are used to increase airway pressure and improve pulmonary function. No systematic review of the effects of PEP in surgery patients has been performed previously. The purpose of this system...
